Air pollution is a well‑established risk factor for the development of asthma, yet most studies examine pollutants individually, failing to capture the health effects of real‑world pollutant mixtures. Assessing long-term multipollutant exposure is essential for understanding asthma development. To estimate the overall effects of joint exposure to multiple air pollutants on active asthma in adulthood, among individuals free of active asthma at earlier assessments, using quantile g-computation (qgcomp). We analyzed 5,299 adults from the Respiratory Health in Northern Europe (RHINE) cohort who were asthma-free between 1990 and 2010. Active asthma was defined at the 2020 follow-up as asthma attacks and/or medication use in the past 12 months. Residential exposures to PM2.5, NO2, and O3 were modelled for 1990, 2000, and 2010. All nine exposure-timepoint combinations were included in a single qgcomp model, adjusted for age, sex, BMI, smoking, education, and study center. In 2020, 312 participants (5.9%) had active asthma. A simultaneous one-quartile increase across all nine pollutant timepoint components was associated with higher odds of active asthma (OR 2.24, 95% CI 1.33–3.78). NO2 and O3 received the largest and most stable positive weights within the mixture model, particularly at the 2000 exposure wave. Sensitivity analyses showed consistent positive associations for 1990 and 2000 exposure mixtures, while estimates for 2010 were attenuated. Long-term exposure to multiple air pollutants in adulthood was associated with higher odds of active asthma by the 2020 assessment. Quantile g-computation is a valuable framework for assessing environmental mixtures and supports the need for multipollutant approaches in prevention.

Background Few studies have considered the complex interplay of the urban exposome in association with multiple respiratory outcomes during infancy. Aim Utilizing an outcome-wide exposome approach, we aimed to assess associations of pregnancy urban exposome with offspring wheezing and chest infections at different time points within the first 18 months of life. Methods The analysis included data from 1032 mother–child pairs from the Barcelona Life Study Cohort (BiSC) (2018–2021). In total, 44 urban exposome factors were assessed during pregnancy, including air pollution, noise, temperature, humidity, green and blue spaces, and socioeconomic and lifestyle factors. Wheezing and chest infection were evaluated simultaneously at 2, 6, 12, and 18 months. We applied mixed-response sparse reduced-rank regression (MsRRR), with resampling procedures, adjusting for potential confounders. This many-to-many modelling approach identifies exposures concurrently associated with multiple interrelated outcomes. Results We found 13 exposures consistently associated with wheezing and chest infection across four time-points. Maternal education was the most consistent protective factor with odds ratios (OR) ranging from 0.65 to 0.81 for university and 0.88 to 0.96 for secondary education (vs. primary education) (all p < 0.05). Other important protective factors were size of nearest green space and maternal light-intensity physical activity. NO2 (OR 1.02–1.08), outdoor temperature (OR 1.02–1.04), and noise annoyance (OR 1.01–1.03) were consistently associated with increased risk. Area-level socioeconomic status indicators showed inverse associations. Conclusion Pregnancy urban exposome may influence both wheezing and chest infections in infancy. Identifying key determinants through an outcome-wide exposome approach can inform targeted public health interventions towards more holistic urban planning strategies.

Background: Air pollution and greenness impact respiratory health, but intergenerational effects remain unclear. We investigated whether pre-conception parental residential exposure to air pollution and greenness at age 20–44 years is associated with offspring asthma outcomes in the Lifespan and inter-generational respiratory effects of exposures to greenness and air pollution (Life-GAP) project. Methods: We analyzed data on 3684 RHINESSA study participants born after the year 1990 (mean age 19, standard deviation 4), offspring of 2689 RHINE study participants. Modelled annual concentrations of particulate matter (PM2.5, PM10), nitrogen dioxide (NO2), elemental carbon (EC), and ozone (O3), and greenness (Normalized Difference Vegetation Index, NDVI) were assigned to parental residential addresses in 1990, corresponding to 1–18 years prior to birth (mean: 6 years, SD: 5). We analyzed associations using generalized structural equation modelling (GSEM), with cluster-robust standard errors allowing for intra-family correlation, while adjusting for potential confounders. Results: Among offspring participants, 18% reported lifetime asthma, 9% active asthma, 8% asthma medication, 5% asthma attacks, and 37% any asthma symptom. An interquartile range (IQR) increase in parental residential NDVI exposure was associated with less lifetime asthma (OR = 0.79, 95%CI: 0.64, 0.98 per 0.3 units). Similar associations were observed for active asthma and asthma medication use. Associations of air pollution with asthma outcomes were inconclusive. Conclusion: Parental exposure to residential green spaces before conception was associated with lower asthma risk in offspring. Urban planning policies prioritizing green spaces may be a key public health intervention for future cities.

Background: Lower birth weight and preterm birth may increase the risk of adverse health outcomes later in life. We examined whether maternal exposure to air pollution and greenness during pregnancy is associated with offspring birth weight and preterm birth. Methods: We analyzed data on 4286 singleton births from 2358 mothers from Respiratory Health in Northern Europe, a prospective questionnaire-based cohort study (1990–2010). Mixed-effects regression models with random intercepts for mothers and centers were used to estimate the association of exposures to particulate matter (PM2.5 and PM10), nitrogen dioxide (NO2), ozone (O3), black carbon (BC), and greenness (Normalized Difference Vegetation Index in 300m-buffers [NDVI300m]) with birth outcomes, adjusting for potential confounders. Results: Median (interquartile range [IQR]) exposures to PM2.5, PM10, NO2, O3, BC, and NDVI300m during pregnancy were 8.4(5.0) µg/m3, 14.4(8.3) µg/m3, 14.0(11.0) µg/m3, 54.7(10.2) µg/m3, 0.47(0.41) µg/m3, and 0.31(0.20), respectively. IQR increases in air pollution exposures during pregnancy were associated with decreased birth weight and the strongest association was seen for PM2.5 (−49g; 95% confidence interval [CI] = −83, −16). However, O3 showed an opposite association. IQR increase in NDVI300m was associated with an increase in birth weight of 25 g (95% CI = 7, 44). Preterm birth was not associated with the exposures. Conclusion: Increased greenness and decreased air pollution may contribute to healthier pregnancies and improve overall health in the next generation. This emphasizes the need to adopt policies that target the reduction of air pollution emissions and exposure of the population.

O₃ viste motsatt assosiasjon. PM10 over WHO 2021 AQG-nivåer var sterkest assosiert med redusert fødselsvekt. I kontrast var NO₂ (OR 1,02-1,08), utetemperatur (OR 1,02-1,04) og støyplager (OR 1,01-1,03) konsistent assosiert med økt risiko.
